Volunteer Agreement & Waiver
This Volunteer Agreement & Waiver (“Agreement”) is entered into between [Unified Motion, Inc.] (“Organization”) and the undersigned volunteer (“Volunteer”). This Agreement describes the terms and conditions of volunteer service and releases the Organization from certain liabilities.
I. Volunteer Agreement
A. Volunteer Requirements
Before beginning any volunteer activities, I agree to meet the following requirements:
Age Requirement: I am at least 18 years old (or have parental/guardian consent if under 18).
Background Check: I will complete any required background check, criminal history screening, or reference checks, if requested by the Organization.
Training & Orientation: I will attend mandatory training sessions or orientation meetings as specified.
Health & Fitness: I confirm that I am physically and medically able to perform the duties assigned, and I will notify the Organization of any medical conditions that may affect my participation.
Documentation: I will submit all required forms, waivers, and documentation (e.g., liability waiver, confidentiality agreement) prior to volunteering.
B. Role & Responsibilities
As a Volunteer, I agree to:
Accept assignments and perform duties as requested by the Organization.
Carry out volunteer tasks responsibly, reliably, and to the best of my ability.
Follow all instructions, policies, procedures, and safety guidelines provided by the Organization.
Communicate promptly about my availability and notify the Organization in advance if unable to fulfill an assignment.
Maintain professional conduct and represent the Organization positively at all times.
Respect the privacy and confidentiality of any information encountered during volunteer service.
C. Code of Conduct
Volunteers shall:
Treat all participants, staff, and fellow volunteers with respect, courtesy, and dignity.
Refrain from discrimination, harassment, or abusive language or behavior.
Abstain from alcohol, illicit drugs, or other impairing substances while volunteering.
Promptly report any concerns, unsafe conditions, incidents, or injuries to a supervisor.
D. Media & Promotional Release
I grant the Organization permission to photograph, record, and use my image, voice, and likeness in any media format (print, digital, or otherwise) for promotional or educational purposes, without expectation of compensation.
E. Confidentiality
Volunteers must not disclose any confidential, proprietary, or personal information obtained during service to third parties, during or after the term of volunteering.
II. Waiver & Release of Liability
Assumption of Risk. I understand that volunteer activities may involve inherent risks, including physical injury or property damage, and I accept all such risks.
Release & Waiver. On behalf of myself, my heirs, and personal representatives, I hereby release, waive, discharge, and covenant not to sue the Organization, its affiliates, directors, officers, employees, agents, and volunteers from any and all liability, claims, demands, or causes of action arising out of or related to any loss, damage, or injury (including death) that I may sustain while volunteering.
Indemnification. I agree to indemnify, defend, and hold harmless the Organization from any claims, liabilities, damages, or expenses (including attorney’s fees) arising from my negligent or intentional acts or omissions.
Emergency Medical Treatment. In the event of an injury or medical emergency, I authorize the Organization to secure appropriate medical care on my behalf and agree to be financially responsible for any costs incurred.
III. Term & Termination
This Agreement is effective upon the date signed below and remains in effect for the duration of my volunteer service. The Organization may terminate my volunteer status at any time, with or without cause.
IV. Acknowledgment & Signature
By signing below, I acknowledge that I have read, understand, and agree to be bound by this Agreement. I certify that I meet the volunteer requirements and that I sign this Agreement voluntarily.
